Attached to the PIAA Physical Form is the Fleetwood Student-Athlete Checklist. Please make sure every part of this checklist is completed before turning in paperwork. If any section is not completed, your student-athlete will not be officially added to the team roster until the missing paperwork is completed. Please take note this is a new packet starting for Fall 2023. ALL athletes, both middle and high school, will be required to complete the ImPACT Baseline prior to being added to the roster. Please see the instructions on the front page (Athlete checklist) and on the last 3 pages (ImPACT Instruction Sheet & Acknowledgement Form).
