In order to try-out, play, or participate, your daughter must have a physical and have turned in a completed PIAA physical form (CIPPE) to the Athletic Director, Matt Diehl, and which has been approved by the athletic trainers. This form is found on the Fleetwood Area School District website under Departments; Athletics; and Forms (found on right drop down menu).

 Physicians request short-sleeved shirts, shorts, and glasses or contacts to be worn for the physical.

If you are having a physical completed by your family physician, please notify the Athletic Department of your intention before 2/13/25. 

If you desire to have your daughter receive a physical through the school, complete sections 1 thru 6 of the CIPPE form AND the last page of the Athletic Handbook (also found on-line under Departments; Athletics; and Forms). These must be turned in to the Athletic Department.

All CIPPE forms will be due by 2/27/25 for evaluation by the athletic trainers. NOTE: all participants must be approved prior to the start of practice on March 3, 2024. Approval time required is a MINIMUM of 24 hours after submission of the CIPPE form. 

Did your daughter play a fall or winter sport?

     Yes. Then, a recertification is all that is needed. So, before TBD, submit a completed Section 7 of the CIPPE form to the Athletic Department Office for approval.
